InfoDigital Vision Cards Only
Plan members will not be issued a physical ID card from UnitedHealthcare Vision. You may log in to the website, www.myuhcvision.com, and print an ID card for yourself and/or eligible dependent(s) by selecting “print ID card”, and the desired dependent from the drop-down menu. ID cards contain Member ID numbers and are required for doctors’ visits and billing purposes.
In-Network Benefits
When using a network provider, participants and dependents are eligible for the following:
Comprehensive Vision Examination: 100% covered, once every 12 months. A vision examination is provided by a UHC Vision network optometrist or ophthalmologist after a $10 copayment.
Eyewear: The plan’s $20 materials copayment is a single payment that applies to the entire purchase of eyeglasses (lenses and frames), or contacts in lieu of eyeglasses. After the materials copay, the plan provides the following vision benefits.
Patient Options: Should you choose patient options not covered by the program such as tints, progressive lenses, scratch, UV, and anti-reflective coating, you will be able to purchase these options at a significant discount. This additional benefit may save you 20% to 40% off of retail on cosmetic lens options and lens upgrades.
Laser Eye Surgery: UnitedHealthcare participants receive access to discounted refractive eye surgery procedures from numerous provider locations throughout the United States. To find a participating Laser Eye Surgeon in your area, visit our web site at www.myuhcvision.com. UnitedHealthcare Vision had partnered with the Laser Vision Network of America (LVNA) to provide our members with access to discounted laser vision correction providers. Contact Lens Allowance Cost Breakdown: The $150 contact lens allowance applies to the fitting/evaluation fee as well as the purchase of contact lenses. For example, if the fitting/evaluation fee is $30, the participant will have $120 towards the purchase of contact lenses. At some retail chain locations the allowance may be shared between the examining physician and the optical store. Toric, gas permeable, and bifocal contacts are all examples of non-covered contacts and are applied to the $150 in-network non-covered contact lens allowance.
Excluded Services and Materials: The following services and materials are excluded from coverage under the Policy: Post cataract lenses; Non-prescription items; Medical or surgical treatment for eye disease that requires the services of a physician; Workers’ Compensation services or materials; Services or materials that the patient, without cost, obtains from any governmental organization or program; Services or material that are not specifically covered by the Policy; Replacement or repair of lenses and/or frames that have been lost or broken; Cosmetic extras, except as stated in the Policy’s Table of Benefits.
Out-of-Network Benefits
Benefits, allowances, and eligible reimbursements will be reduced when using an out-of-network vision provider. If you choose a non-network provider, you will need to send itemized paid receipts, with the primary insured’s Social Security number, patient’s name and patient’s date of birth to:

exclamationOut-of-Network Reimbursement Limitations
Receipts for services and materials purchased on different dates must be submitted together at the same time to receive reimbursement. Receipts must be submitted within 12 months of the date of service. UnitedHealthcare will reimburse you according to the schedule shown in the Vision Benefit Plan Design Summary document.

Carefully review this material to understand the plan design and applicable co-pays.
Find a provider
Locate a provider by logging on to www.myuhcvision.com and selecting the provider locator option. Participants may also contact UHC Vision’s 24-hour, toll-free Interactive Voice Response system at 1-800-839-3242 to locate a nearby provider. See additional instructions below.
Schedule an appointment
Once a provider has been chosen, simply call the provider directly to schedule an appointment. Provide the primary insured’s Member ID, the patient’s name and date of birth, and state that UHC Vision Plan coverage is in effect.
Receive services
The network provider, a state-licensed optometrist or ophthalmologist, will perform a complete eye examination, including a case history of the patient, an examination for eye pathology and abnormalities, visual analysis (refraction), confrontation visual fields testing, condition diagnosis, and prescription determination.
Choose eyewear
If prescription eyewear is necessary, the UHC Vision provider will write a prescription, assist with eyewear selection and order the eyewear. The UHC Vision provider will telephone the participant when the eyewear arrives. Eyewear is dispensed at the provider’s office to ensure optical accuracy and proper fit.

With UnitedHealthcare, you are able to choose from participating optometrists, ophthalmologists, and retail chain providers. Prior to enrolling in or using the UnitedHealthcare vision care plan, if you would like to identify a network provider, call UnitedHealthcare’s Provider Locator Service at 1-800-839-3242 and follow the voice prompts:
- Enter your Social Security number.
- Enter your zip code.
- After each entry, the system will repeat what you have entered and ask that you “Press 1” if correct, or “Press 2” if incorrect.
- The system will then identify up to three network providers in your area.
- If you wish to hear the selections again, press 1. To enter another five-digit zip code, press 2.
Prior to using your benefits at a network provider, please call the provider and make an appointment. Please inform the provider that you are UnitedHealthcare participant. This system will allow you to find providers in your area prior to enrolling in or to using the UnitedHealthcare Vision Care Plan OR visit UnitedHealthcare’s web site and provider locator: www.myuhcvision.com
