InfoDigital Vision Cards Only
Plan members will not be issued a physical ID card from VSP. Though an ID card is not required to receive services from VSP providers, you can print a copy for your records from your VSP member account. Visit ua.vspforme.com, register a new account, then select “View Member ID Card” for yourself and your eligible dependent(s).
In-Network Benefits
When using a network provider, participants and dependents are eligible for the following:
You’ll continue to have access to quality eye care and eyewear at low out-of-pocket costs with VSP. Plan enhancements include lower premiums with a 4-year rate guarantee, plus higher allowances for frames and contacts with new plan design features. VSP will also offer a connected claims feature to assist with substantiation of FSA expenses. View detailed benefit plan information.
Exclusive Member Extras: With VSP, you and your family are also eligible to save on everyday products and services that fit your needs beyond vision care—like discounts on fitness, nutrition, hearing aids, prescription drugs, and diabetes resources. Learn more.
Setup Your Online Account
You’ll get the most out of your benefits when you log in and view your personalized benefits, look at your claim history, print your Member ID Card, and more. Visit https://ua.vspforme.com/ to create your account and get started!
Find Your Provider
You can choose from participating optometrists, ophthalmologists, and retail chain providers. VSP prioritizes local, private practices so certain “big box” retailers like Target and Costco will be out-of-network and cost more to use. Prior to using the VSP vision care plan, if you would like to identify a network provider, visit https://www.vsp.com/advantage to search by zip code or street address.

Schedule an Appointment
Once a provider has been chosen, simply call the provider directly to schedule an appointment. Provide the primary insured’s Member ID, the patient’s name and date of birth, and state that VSP Vision Plan coverage is in effect.
Receive Services
The network provider, a state-licensed optometrist or ophthalmologist, will perform a complete eye examination, including a case history of the patient, an examination for eye pathology and abnormalities, visual analysis (refraction), confrontation visual fields testing, condition diagnosis, and prescription determination.
Choose Eyewear
If prescription eyewear is necessary, the VSP Vision provider will write a prescription, assist with eyewear selection and order the eyewear. The VSP Vision provider will telephone the participant when the eyewear arrives. Eyewear is dispensed at the provider’s office to ensure optical accuracy and proper fit.
