University-Paid Long Term Disability Insurance

The University provides, at no cost to eligible employees, long term disability (LTD) insurance that provides for income replacement/salary continuation at 66 2/3% of current salary (not to exceed $10,000 per month) after a 90-day waiting period if the employee is unable to return to work due to “total disability” from a non-work-related injury or illness. If your claim is approved by The Standard, benefits are payable on the 91st day from the date of disability. After 90 days of benefit payments, the plan changes from 66 2/3% to 60% of pre-disability earnings reduced by deductible income and continues at that rate for the duration of the claim. Maximum Benefit Period: If you become disabled before age 62, LTD benefits may continue during disability until you reach age 65. If you become disabled at age 62 or older, the benefit duration is determined by your age when your disability begins up to a maximum of 3 years and 6 months.

When Benefits End: LTD Benefits end automatically on the earliest of:

  • The date you are no longer disabled.
  • The date your maximum benefit period ends.
  • The date you die.
  • The date benefits become payable under any other LTD plan under which you become insured through employment during a period of temporary recovery.
  • The date you fail to provide proof of continued disability and entitlement to benefits.

University-Paid Group Term Life Insurance

The University provides, at no cost to eligible employees, a group term life insurance plan from The Standard that pays your beneficiary a designated amount upon your death. The Group Term Life coverage comes with a special feature called the Accelerated Benefit Option. This option allows you to access up to 80% of your group term life insurance proceeds to a maximum of $240,000 should you become terminally ill and are diagnosed with less than twelve months to live. Coverage includes complimentary access to the Life Services Toolkit. The coverage amount varies with annual salary:

University-Paid Accidental Death & Dismemberment Plan

The University provides, at no cost to eligible employees, an Accidental Death & Dismemberment (AD&D) policy from The Standard in the amount of $22,500 payable if death was caused by an accident. In instances where the employee’s death was caused by an accident, the beneficiary would receive both the University-Paid Group Term Life and AD&D benefit. AD&D also pays a benefit if a serious injury results in dismemberment (i.e., if you lose a limb or the ability to see, you may be paid a part of your benefit). Coverage includes complimentary access to the Life Services Toolkit.

Additional coverage can help defray the loss of income and help your family maintain the household in the event of your death. Eligible employees have the option of purchasing additional term life insurance of 1 to 5 times your basis annual earnings (BAE) rounded to the nearest $1,000 OR in $50,000 increments, to the lesser of 5x BAE and a maximum of $1,400,000. For your spouse, you may elect the lesser of the employee’s voluntary life amount and $150,000 (in $10,000 increments). For your dependent child(ren), you may elect $10,000 for children up to age 26. Subject to the following rates:

Guaranteed Issue: The policy is guaranteed to be issued for new employees, without any evidence of insurability, who elect the lesser of 3 times their salary or $500,000 if the application is approved during the first 60 days of employment. Guaranteed issue for spouse life is $30,000; any excess will be subject to medical underwriting.

Accelerated Benefits Option: You can receive up to 80% of your Voluntary Life insurance coverage amount to a maximum of $500,000 in the event that you become terminally ill and are diagnosed with less than 12 months to live.

Waiver of Premiums for Disability: Your life insurance coverage can be continued at no cost to you should you become unable to work due to total disability.

This benefit provides eligible employees additional Accidental Death & Dismemberment (AD&D) coverage with a minimum coverage of $25,000. The maximum amount you can receive is the lesser of 10 times base annual earnings and $500,000. Additional coverage can help defray the loss of income and help your family maintain the household in the event of your death. New employees must enroll in the plan during their first 60 days of employment. You may choose coverage for yourself, your spouse and/or your dependent child(ren) under the Family Plan.

  • If you cover your Spouse and Dependent Child(ren): Spouse = Covered at 40% of your coverage amount; Child(ren) = 10% of your coverage amount
  • If you cover Spouse only = 50% of your coverage amount
  • If you cover Child(ren) only = 15% of your coverage amount

Voluntary Short-Term Disability Insurance

Beginning January 1, 2020, UA will offer a voluntary short-term disability policy sponsored by The Standard. Short-term disability pays 60% of your salary (up to a maximum weekly benefit of $1,000) if you become temporarily disabled, meaning you are unable to work for a short period of time due to illness/pregnancy or accident. All regular, full-time employees are eligible to enroll in this post-tax, employee-paid benefit. There are two plan options with different benefit waiting periods – 14 days or 29 days; see details below.

New employees will have 60 days from date of hire to enroll without penalty. Employees who enroll after this initial new hire window will be subject to a late enrollment penalty with a 60-day extended benefit waiting period for the first 12 months. In order to receive benefits once disabled, the employee requesting short-term disability benefits must be in an unpaid status with The University (i.e., not currently receiving OJI benefits or sick leave).
