Retail and Home Delivery Copays – This document lists the various copays for generic, preferred brand, non-preferred brand and specialty drugs.

OptumRx Member Portal Registration Guide – Employees will need to use the member information located on your OptumRx ID card to register on the website.  Once you have registered, the member portal will allow you to view claim history, calculate copays, locate pharmacies and look-up specific prescription drugs. Access the OptumPx member portal. 

UA Formulary Reference Guide – This document lists the prescription drugs covered and if any prior authorizations, quantity or gender limits apply.

2018 Formulary Exclusions – This document lists specific prescription drugs excluded from the 2018 Premium Formulary and provides alternatives.

OptumRx Utilization Management – The following documents list the non-specialty prescription drugs with utilization management programs to help ensure appropriate medication use and lower overall health care costs.
